What is support coordination?
A support coordinator plays a vital role in assisting individuals with disabilities or special needs. Their primary responsibilities include:

  • Understanding the client’s needs: They work closely with clients to understand their goals, needs, and circumstances.

  • Plan and coordinate supports: They help clients understand their plans (such as an NDIS plan) and coordinate the various supports and services required.

  • Connect with services: They link clients with appropriate supports and services, both within the NDIS and in the broader community.

  • Establish and maintain supports: They assist in establishing supports and ensuring they continue to meet the client’s needs.

  • Capacity building: They coach and mentor clients to build their independence and skills to understand, navigate, and implement their plans.

  • Crisis management: They prepare for and manage unexpected events and crises, ensuring the client continues to receive necessary services.

  • Advocacy: While they do not act as independent disability advocates, they can help clients understand when such services might be needed and how to access them.
